Baclofen is a derivative of GABA and a selective agonist of the GABAB receptor. The GABAB receptors are G-protein-coupled metabotropic proteins responsible for depression of synaptic transmission and inhibition of neuronal excitability, distinct from the GABAA and GABAC ionotropic receptors responsible for fast neuronal response. As one of the only identified molecules with specificity for the GABAB receptor, Baclofen has been widely employed in elucidating the function and behavior of these receptors. Agonism of GABAB by Baclofen was initially identified to produce muscle relaxation and relief of muscle spasm. Modulation of the GABAB receptor by agonists has been correlated to reduction of reinforcing effects in addictions. Recently the GABAB receptors have been identified as modulators of transient lower esophageal relaxations, a molecular underpinning of gastroesophageal reflux disease. Baclofen provides a prototypic structure for design of GABAB agonists to differentially modulate the behavior of the GABA system.
